A Work Order Form can be created to allow a patron to submit a request to have an item fixed. When a patron submits a Work Order, they include a description and location of the problem, the problem type, and a contact phone number and email address. Once a Work Order has been submitted, a patron can view the status of the Work Order.
Note: For ResCenter users to have access to the Forms page, they must have access to the Administration.Term User Roles Permission. Refer to the Security Setup section for information about configuring User Roles.
Work Order Form Implementation
When a form is configured as a Work Order, it will be available to patrons who have a contract assignment for the Term associated with the form.
When a patron creates a Work Order, the patron must include a description of the issue and the location (i.e. building, floor, room). Optionally, the patron can select a Work Order Type (defined via Setup > Maintenance > Work Order Types), enter a Title, and upload a picture. Finally before submitting the Work Order, the patron can enter an email address and phone number, and elect to be contacted with updates.

Patrons can submit a Work Order Form if they have an assignment contract for the Work Order’s defined Term.
